Big Bad Pharma

1 / 8

12,792 items. One unbelievable mess. You.

Clock in. Stock the shelves. Climb the ladder of an industry that maybe shouldn't exist.

Big Bad Pharma is a cosy first-person pharmacy-stocking sim with a wicked streak. On the surface it's everything you want from a tidy little restocking game: neat shelves, a quiet shop to run, and that deeply pleasing click of the last box sliding into place. Underneath, it's a cheeky satire of corporate healthcare, and the further up the ladder you climb, the more you might want to ask yourself whether it was worth it.

Every shift starts the same way: shelves stripped bare, product everywhere, and you.

Every single one of the 12,792 items on that floor has a home. A specific shelf, in a specific bay, in a specific aisle, and the shop isn't finished until every last one of them is in it. Pick things up, carry an armful, and slot them into place.

Categories run from digestive relief to migraine remedies to a genuinely suspicious quantity of allergy medication, all of it wearing a label that's almost a brand you'd recognise. Legally speaking.

Forgotten where the antihistamines live? There's a catalogue book in your pocket. Whether you're a methodical shelf-perfectionist working one bay at a time or a chaotic box-flinger lobbing paracetamol across the room, the shop is yours to run.

Every shift starts the same way — empty shelves, a full stockroom, and you. Sort products into the right aisles, from digestive relief to migraine remedies to a suspicious number of allergy medications. Keep things tidy, keep things moving, and try not to lose an item along the way. Whether you're a methodical shelf-perfectionist or a chaotic box-flinger, the shop is yours to run.

Nobody climbs the corporate ladder on foot forever.

Behind the staff door there's a beige desktop PC running an operating system that time forgot. That's the Rx Upgrade Hub, and it's where your wages go. Carry more in one trip. Move faster between aisles. Jump higher, throw further, and unlock the four tools that turn a long shift into a short one:

Locate : light up every stray item of a category, wherever it's hiding

Auto Shelve : send what's in your hands straight to where it belongs

Magnet : pull nearby product to you without bending down once

Sweep : gather up a whole patch of floor in a single pass

Max everything out and you become a truly well-rounded (and slightly terrifying) employee of the month.

There's more to this pharmacy than restocking. Twenty-one achievements trace your journey from bleary-eyed newcomer to something a little more... corporate. There are things written on mirrors. There's a radio out back with four decades of music on it. There's a toilet, and yes, it flushes.

Clock out at the end of a shift and the numbers come up: items shelved, sections completed, time on the floor, and a running record of every shift you've ever worked. Then the shop refills, the day ticks over, and it all starts again, which is either deeply comforting or the entire joke, depending on how you're feeling.

Big Bad Pharma is warm, funny, satisfying to play, and just self-aware enough to make you smirk. It's a game about small satisfactions and big questions, made by one person with a sharp sense of humour and far too much time spent thinking about shelf geometry.

Plays on mouse and keyboard or controller, in five-minute bursts or three-hour ones.

Come for the shelves. Stay for the slow moral decline.

Love sorting games. This ones a big issue. - There is no descriptor for the items on the ground like it was displayed in the preview on the steam page, I have no idea what I am picking up. - A lot of the items look the same so I am relying heavily on the green box to appear to place them, but that also means you can't incorrectly place items. The box can also be weird sometimes and not show up unless you are ON TOP of it. - Some of the shelves display the wrong numbers and you have to take all the items off and put them back on to get them to display the correct ending number so the sign will turn green. - The abilities feel extremely useless. They don't feel like upgrades and they don't do much. (I also played with none of the abilities unlocked and didn't receive the achievement for it so I don't know if it's bugged or it counts the jump and throw as abilities?) - Towards the end when I would put items on the floor they would keep setting themselves back to their original floor location from the very start of the game - Sometimes the shelves display a 1/1 for the item and wont let you place another item on the shelf. I had to go back and alphabetically order the cardiovascular section just to beat the game cause the numbers kept messing up. - the floor layout for the shelves is extremely annoying to me. The cold and flu section is technically in three different sections instead of making it an actual isle to itself. The game was a great concept and I loved sorting all the items but it became way more tedious than it needed to, and I even went the route of not using the abilities the first time for the achievement that I never received.

Not even close to ready. The game still plays behind the guide book (The guide book which is inaccurate btw) and the shelves for some categories are split BEHIND their row and others are on random walls. It's impossible to sort the shelves in a satisfying way in the alphabetical order.

I've been following this game since it could be wish listed. The developer really stayed on top of bugs and qol adjustments. They made sure to take out the Ai and also did all the drawings by their own. The fact that all this was taken into consideration made the game stand out. The amount of items to put up and the chill atmosphere is a good way to distract from those hard work days or whatever you got going on. Give it a shot organizing game lovers.

I want to say I can recommend this game, but I honestly can't. The concept is fun and I am in total support of the creator removing AI (I wouldn't have bought the game if there was any AI involved), but there is one very big issue that makes it difficult for me to play this game for extended periods of time. All of the items look far too similar. Some of them are even the exact same color as well. The titles are helpful, of course, but it means that when sorting it's very difficult to tell at a glance what is meant to go where without reading each individual label, which can make the game headache inducing. This is worsened by the fact that there's no power-up to tell you where products need to be shelved, so there isn't anything you can do but go through each shelf to find the right one. I would recommend waiting until the dev makes the products a little more distinct before purchasing this game. It also takes a very long time to gain abilities. I'm six hours in and still haven't gotten many unlocks, and the unlocks you do get don't feel very powerful. But honestly that part could just be a me issue, and isn't my main problem with the game.

I honestly can't recommend it right now. Its not that I don't enjoy it overall, but it just pushes too far in to the tedious area of these kinds of games. Normally in games like this, you start our doing things manually to unlock upgrades that reduces how tedious the game is. This game, however, has upgrades that don't do much at all. For example, if you unlock the auto-place ability it starts letting you auto place 4 items. It doesn't go by how much hold space you've unlocked. Each upgrade after only unlocks 1 additional slot. So even if I have 10 inventory spots, I'm only able to auto-place 4 and then 5 items. It costs me $900 to get the extra slot. Then the items tend to look very similar (even snacks look like pill boxes), and many items have multiple shelves of the same items so sorting is a bit annoying. I just feel like the game could use some QOL tweaks to improve how it feels to play it. There is none of that feeling of getting quicker at placing items and gratification when unlocking new abilities right now.

When i first started to play i loved it! But already a little in the game there are items missing for spesific places. Some I can't find. Its nowhere to be seen and when i press the ability to have it teleport to me then there is nothing showing up. And it said clearly 23/25 items yet i cant find it. My friend has had the same issue with not finding the items and cant finish the game doing so. And its a bit annoying that we cant finish the game when it had so much potential

i like that this game doesn't use ai however it's very difficult to tell what goes where unless you walk around like an idiot looking for the green box on shelves. requested refund.
